I absolutely concur with removing duplicate maps; particularly favorites that get voted often like dolls and innomotel. There have been more than a couple close calls where we have almost gone from ttt_dolls to mc_dolls and the same for innomotel. Given how both are generally inferior versions...

For me what fixed this was running "gl_webview_force_latest 1" in my console. Give this a shot next time you're on. If that doesn't work then I'm not sure what it may be.

This guy kill binded and hid his own bomb under his corpse and I picked it up with less than a second to go and got an explosion jumpscare. +1 (Although I will say this may be intended - players bomb not despawning if they kill bind / die from fall dmg, etc.)

Title, santa hat is basically stuck on my head (other players have corroborated). It remains even after holstering & unholstering. Apparently it doesn't appear on ragdolls.
