1 person starts as a stronger (maybe more hp and faster) infected. Anyone they kill becomes a regular infected. The goal is for the survivors to hold out as long as possible and the infected goal is to turn everyone infected. Infected could be equipped with an instant kill knife and survivors could spawn with random guns. I think it would be really fun depending on the map.
