Invisible ladders / doors

I haven't had the problem myself at any point, but several people frequently mention and complain about invisible doors and ladders. I couldn't see any other current reports on it, so I'm here to rectify that. The problem applies across maps.
Idk how to replicate, since I've not had the problem.

I've had this happen to me on Community Pool.

If I notice it happening for other maps I'll note it in this post.
